A coordinating physician
A functional medicine physician or concierge internist who will own your record, order baseline testing, and arbitrate conflicting advice later. Choosing this person first prevents a pile of disconnected opinions.

A genetic counselor — before you test
If APOE or a dementia panel is on the table, counseling belongs before the blood draw, not after. This is also when you secure life, disability, and long-term care coverage, which GINA does not protect.

Whoever addresses your largest modifiable risk
For most people that is sleep or metabolic health. A sleep specialist for suspected apnea, or a brain health nutritionist if lipids and glucose are the problem. One lever at a time so you know what worked.

A neurologist for diagnosis
If symptoms are present rather than theoretical, a behavioral neurologist and biomarker workup should come early. Prevention-focused care does not replace a diagnostic answer.

A coach and a therapist
The two most underused roles. A health coach closes the gap between plan and behavior; a therapist handles the anxiety, family conversations, and caregiving load that risk information creates.
